The Oaks is an independent retirement community that is a part of the Menorah Park Campus. Residents at The Oaks are active seniors that enjoy the convenience
of having services like meals and housekeeping available to them. As needs change some residents bring in personal care assistance. As a continuum of care, it is our policy that residents at The Oaks be alert and self directing and have the ability to safely evacuate the building.


All new residents are invited to meet with one of our Campus nurses prior to admission
to have a per-admission discussion outlining basic health issues.
The purpose of this discussion is to give us appropriate information that we can pass along in the event
of a health related emergency as well as to confirm that our new resident is safely able
to maintain an independent lifestyle.
Prior to admission we also require a prospective resident to complete an Application
for residency.
You can download a copy of our application from this site. As a part of a continuing care community this information will be helpful in the event of a change of
status and / or residency within our campus.

  • Both Federal and State laws impose severe penalties for obtaining Medicaid fraudulently. Please note that Federal Law prohibits the transfer of most assets for 60 months prior to applying for Medicaid. This signed application represents accurate disclosure of all assets. This Menorah Park Application for Residency may be used in the future to substantiate any future Medicaid application. I hereby declare that the application represents true and accurate information; I understand that I may be asked to provide supportive documentation to verify this information. Menorah Park reserves the right to have this application updated at lease renewal or prior to a move into the Jewish Health and Rehabilitation Center.
