If you or a loved one needs at-home assistance, our licensed agency will create a plan that fits your individual needs, including:

Our registered nurses can help you decide which services are right for you and our home health aides facilitate daily activities and personal care.  We provide an on-call registered nurse 24 hours a day to answer any and all questions you may have.

At-Home Medical Care

Types of professional visits

• Initial nursing assignment
• Skilled nursing visit
• Patient Review Instruments (PRIs)

Services we offer:

• Case Management: Evaluate and coordinate all aspects of needed care and daily living.
• Nursing: Performed one-on-one by a registered nurse, we help you decide which services are best for you. We also provide shift nursing and communicate with your doctor regarding your progress with follow-up assessments as changes arise.
• Medication Management: Provide weekly pre-pouring of medication and daily reminders or administration and inventory management.

Companion/Homemaker Care

Social / Recreational

• Provide companionship: talking, listening
• Play games, cards
• Assist with crafts, hobbies
• Walks

Light Housekeeping

• Clean bathroom/ kitchen
• Wash dishes
• Change linens

Nutrition
